Telemedicine consent
Last updated: April 11, 2026
This notice explains how care may be delivered remotely through Longegra. It is not a substitute for in-person care when your clinician determines an exam or emergency services are required.
Nature of telemedicine
Telemedicine uses technology (including secure messaging, questionnaires, and video when offered) so licensed clinicians can review your information, provide guidance, and coordinate treatment when appropriate. You understand that virtual visits have limits: not every condition can be diagnosed or managed without an in-person evaluation or tests ordered by your clinician.
Your responsibilities
- Provide accurate, complete health information and update us if anything changes.
- Participate in follow-up questions or visits your clinician requests.
- Seek urgent or emergency care (e.g., chest pain, severe shortness of breath, stroke symptoms) by calling local emergency services—not through this platform alone.
Privacy and records
We protect information according to our clinical data practices and applicable law. You may request access or corrections to your records as described in our Health data processing notice.
Consent
By using Longegra's telemedicine services, you acknowledge that you have read this summary, had the opportunity to ask questions, and agree to receive care through the methods we describe in your visit and intake flow.
Medical disclaimer: Longegra provides a platform for clinical workflows. Your treating clinician is responsible for medical decisions. Nothing on this site replaces individualized medical advice.
